Welcome to Glen Loch Elementary
I collect and enter the registration information for each new student and provide attendance reports as needed.
NEW STUDENT REGISTRATION REQUIREMENTS
1. Birth Certificate
2. Social Security Card (If no card, a state PEIMS # will be assigned)
3. Proof of Residence (Current utility bill, Lease Agreement)
4. Immunization Record
DPT or DT (4 doses) One dose must be after 4th birthday
POLIO (4 doses) One dose must be after 4th birthday
Measles (2 doses) Received after the 1st birthday
Mumps (1 dose) Received after the 1st birthday
Rubella (1 dose) Received after the 1st birthday
Hib CV (1 dose) Received after the 1st birthday
HepB (3 doses)
Varicella (1 dose) (or disease history verification)
5. Copy of Drivers License (adult enrolling the student)
6. Any Legal Documentation
Pre-Kindergarten - the student will need to be 4 on or before September 1, of the current school year. Complete a Pre-Kindergarten application and provide proof of a current month's income.
Kindergarten - the student will need to be 5 on or before September 1, of the current school year.
Students Entering After the First Day of School - Please provide a withdrawal form from the previous school, and a list of all the previous schools attended with the address or phone number.
ABSENCES
ATTENDANCE IS TAKEN AT 9:00 A.M.
When a student is absent please call the school (281)298-4900 or (281)298-4911. The front desk will record the absence information, or you may speak with me. Please provide a written excuse if you don't call concerning the absence.
If a student has an appointment with the doctor before 9:00 A.M. or leaves the building before 9:00 A.M. for a medical appointment and returns to school before the end of the day, please provide a doctor's excuse and complete the HCP Form. The HCP form is at the front desk. This deletes the tardy or absence for the day. If you have questions about this please contact me.
