In order to make registration an efficient process, we ask that you have the following documents and information ready:
Documents required for enrollment in Pre-Kindergarten
Proof of Income
Birth Certificate
Social Security Card
Immunization Records---Complete and up-to-date for age
Proof of Residence (such as a utility bill or rent receipt, etc.)
Custody Papers (signed by a judge, if child is not living with both
natural parents)
Documents required for enrollment in Kindergarten
Birth Certificate
Social Security Card
Immunization Records---Complete and up-to-date for age
Proof of Residence (such as a utility bill, rent receipt, etc.)
Custody Papers (signed by a judge, if child is not living with both natural parents)
Documents required for enrollment in First (1st) through Fourth (4th) Grades

Last school’s name and address and report card if coming from a school not in Conroe ISD.
Withdrawal form if enrolled during current school year
Custody Papers (signed by a judge, if child is not living with both
natural parents)
