To enroll in kindergarten, your child MUST be five (5) years old on or before
 September 1, 2006. You MUST have the following information:

Child's Birth Certificate
We need your child's original state recorded certificate, not a hospital certificate.

Child's Social Security Card
We need your child's original Social Security Card, not a copy.

Proof of Residency
A current utility bill or rental/lease agreement may be used.

Driver’s License

Child's Immunization Records
The immunization record MUST include the following:

	3 doses of Oral Polio or Polio Vaccine, 1 dose given on or after the 4th birthday.
	4 doses of DPT, 1 dose given on or after the 4th birthday.
	2 doses of MMR, 1st dose given on or after the 1st birthday.
	3 doses of Hepatitis "B" for students born after September 2, 1992, given within a 7  month period. The 1st dose must be given prior to the beginning of school.
	For students born after September 2, 1994 - 1 dose of Varicella (chicken pox) vaccine or documentation of having had the disease.
	As of September 1, 2005 for Pre-Kindergarten, P.P.C.D., and Early Childhood:
	· 2 doses of Hepatitis A
	· 4 doses of Prevna/Pneumoccus by 2^nd birthday, if the child does not have all four doses by 24 months then they must have one booster dose between 24 months – 59 months.

Morning or afternoon placement for kindergarten will be determined by address so that transportation can be provided to and from school.
