• Birth certificate
• Immunization records (up to date)
• Proof of address (utility bill in parent/guardian's name or rental/sales agreement)
• Last report card, if available
• Two months recent paycheck stubs, only if registering for PreKindergarten
• Student's social security card or state I.D. card
• Parent's drivers license
• Custody papers, if applicable
