Referral
a. Letter to the Parents of Prospective Homebound

the parent before proceeding to #4.

5. The guidelines for evaluation are as follows:
a. The diagnostician will obtain:
i.
Notice and
Consent for Evaluation (877-106)
ii.
Initial Distribution
of Notice of Procedural
Safeguards
(877-106a)
b. The Full and Individual Evaluation (877-200) will be
completed. The diagnostician may review
existing data, such as testing by the classroom teacher, TAKS/SDAA scores,
attendance records, discipline records, and grades to complete the written
evaluation report.
c. If the letter from the physician indicates a
psychological/emotional disability, the LSSP will review the report, contact
the physician, and make a written recommendation to the ARD Committee.
6. The campus diagnostician/ARD facilitator notifies the
lead homebound teacher to assign a homebound teacher.
7. The lead homebound teacher will request scope and
sequence information from the student’s teachers using 877-944.
8. Campus special education staff schedules the ARD
Committee meeting.
9. The homebound teacher and/or special education staff
will draft IEPs for each subject that may be taught by the homebound teacher.
During the ARD Committee Meeting
1. It
is an ARD Committee’s decision to determine if the student meets eligibility
criteria.
After the Placement ARD Committee Meeting
1. Campus
special education staff will notify the student’s teachers and attendance clerk
that the student has been placed on homebound services as of the initiation of
services date.
2. The
homebound teacher who reports the student’s absences to the attendance clerk
maintains attendance.
3. The
diagnostician or campus staff will distribute the Homebound Classwork
Assignment Sheet (877-946) to classroom teachers to complete. It is important to have completed
assignment sheets available to the homebound teacher by the initiation date of
services.
4. The
diagnostician/ARD facilitator will add the student’s name to the current
special education SEMS roster.
5. The
diagnostician/ARD facilitator will document on a calendar the anticipated date
of the student’s dismissal ARD meeting and plan for it appropriately.
6. IEP
progress reports for homebound services will be sent home with the report card.
Prior to Dismissal ARD Committee Meeting
1. The
diagnostician/ARD facilitator will communicate with the parent regarding the
student’s re-entry to school.
1. The
Homebound Re-entry (877-405) will be reviewed.
2. The
campus staff will receive the Homebound/Hospital Grade Report (877-404a or
404b) from the homebound teacher, if she assigned grades to the student.
3. Grades,
credits and attendance will be addressed at the ARD Committee meeting.
4. IEPs
will be updated and coded.
After
the Dismissal ARD Committee Meeting
The designated campus staff will notify the student’s teachers and the campus attendance clerk of the date that the student will return to school.
